Office Policies & Forms

 

Appointments

Appointments can be made during our office hours by calling (410) 871-0088. Our office hours are Monday through Friday, 8:30 a.m. to 4:30 p.m.

Cancellations

As a courtesy, we request that you make any appointment cancellations at least 24 hours in advance. Patients that are more than 15 minutes late or do not show for their appointments will be charged a fee of $25.

After Hours & Emergency Care

If you are experiencing an emergency, dial 911 or head to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day, 7 days a week at (410) 871-0088.

Prescription Renewals

Prescriptions are renewed during normal office hours. We require 48 hours notice for prescription refills. In some instances, we may require that that patient be seen in our office prior to medication renewal.

Financial

All office visits are payable at the time of service. We accept cash, checks, most major credit cards and debit cards. Please contact our billing office should you have any financial questions or concerns.

For your convenience, you may download and print the patient forms listed below and bring them with you to our office or fax them to us at 410-871-0083 before your appointment.

Note: All patients are required to complete the HIPPA Privacy Policy.
